This verse is taken from the Douay-Rheims Bible:

“And his sweat became as drops of blood, trickling down upon the ground.” – Luke 22:44

If Jesus “sweat became as drops of blood” did he really sweat blood or was his sweat like blood?

I believe that Jesus sweat blood. To get some idea of what incomprehensible sorrow, horror, terror Jesus endured in the Agony, I recommend Anne Catherine Emmerich’s (she is a Venerable, I believe) account. Leave the rest of her work (or the work attributed to her); I’ve been through it, it’s not so good, in my humble opinion. You can find the Agony account at jesus-passion.com and other places on the internet, I think. This reading really started me on the path to understanding the awful hellishness of the Passion. The Agony was incomprehensibly bad, and it would be fitting for things to get worse from there on…

Also, about the Passion, time passes more slowly the more pain one is in. Jesus’ whole life was filled with suffering, of course. In John’s account of the part of the Passion in the garden, it can be read I think that the whole host of hell (the men fall backward at the name of Jesus) comes down on Jesus. “If you seek me, let these ones go… etc. etc.” Jesus taking it all on himself, to save all. I wonder if the young man in a white garment is not, in addition to being a real young man there, a hint about the angels themselves being confounded at the “hour of darkness.”

Someplace I read that when one is under extreme stress the capillaries can rupture and leak blood into the adjacent sweat glands. Thus blood is exuded along with the sweat.

Sorry, it was a long time ago that I read that and I have no guess as to where.
 
It’s just the way St Luke words this particular verse as if he was using a metaphor by saying “sweat became as drops of blood” which kind of confuses me…

Sweating blood (or blood pigment) is a very rare medical condition known as hematidrosis and is usually associated with extreme stress and high blood pressure.

Ver. 44. And his sweat became as drops of blood, &c. This has sometimes happened, though in a lesser degree, to persons under extraordinary grief, if we believe Aristotle, lib. iii. Animanium, chap. xix. p. 891, and lib. de part. Animalium, chap. v. p. 1156. Ed. Aureliæ Allobr. an 1607. — This passage of Christ’s bloody sweat, and of the apparition of the angel, was heretofore wanting in divers both Greek and Latin copies; as appears by St. Jerome, (lib. ii. cont. Pelagianos. tom. iv, part 2, p. 521) and by St. Hilary, lib. x. de Trin. p. 1062. Nov. Ed. It seems to have been left out by ignorant transcribers, who thought it not consistent with the dignity of Christ. But we find it in the above-said place, in St. Jerome, in St. Chrysostom (hom. lxxxiv. in Matt.), in St. Augustine (in Psalm cxl. tom. iv, p. 1564, and in Psalm xciii, p. 1013.) in St. Epiphanius in Ancorato, p. 36, Ed. Petav. (Witham)
 
Note that Saint Luke is reputed to have been a physician. He tends to give accurate details of things like this.

It does not specify. In this case the meaning is the sensible properties were the same as blood. In other words, he was being technically correct. He was not there, he could not test it to see if it was blood. But he knew from eyewitnesses it had the viscosity and color of blood.

I would assume it is blood but technically that is not what he said.

It’s rare but very well documented that this can occur to people in high-stress situations.
