Can every bishop trace back to the apostles their ordination line? Or are there broken historical lines for some bishops and as such they can not trace back their line of ordinations back to the Apostles?

It is my understanding that every Bishop can trace their ordination back to the Apostles.

It is generally the case that the laying on of hands when a new bishop is consecrated is performed by three other bishops. So there would almost never be an instance where there was doubt about succession being present.
