On the topic of apostolic succession, its existence is clear from Acts when the “bishopric” of Judas was filled. I understand that no office should be left vacant, and there were 12 “offices” of bishops in the apostles. When were additional offices added, that didn’t exist before and weren’t vacant, to total the number of bishops we have today?

I believe when would have been the inclusion of Saul, or Paul, he was counted among the Apostles after his acceptance by Peter and the other Apostles, not as a replacement, but as an addition. If you need references let me know, I’ll look them up.

Paul tells Titus to appoint bishops also. The Fathers typically viewed Timothy and Titus as bishops also.
