Does anyone know if women religious orders require full disclosure of a candidate’s past medical records, both physical and psychological? Do you have to sign a release form for both physical and psychological records?

You’ll most likely have to undergo a physical examination and a psychological evaluation in order to be admitted to an order.

For my men’s order I had to get a physical, they provided the forms. I don’t remember what was on those forms regarding medical history. I also had to meet with a psychologist and undergo a battery of psych tests.

I imagine a women’s order is probably similar.
 
I had a very wealthy Order tell me no because past medical history and “financial concerns” in the extremely rare case a past issue came back or affected me in the future. I also had a very financially poor Order accept me, seeing no reason the same prior medical issue should keep me away.

I have no doubt God work’s through this process, to guide someone in whatever vocation He’s calling them to.
