Baptismal incorrect date of birth

Paano po ba ang pwede gawin para maitama ang date of birth sa baptismal? Ang nso or psa record of date of birth is April 10, 1980. Ang nakalagay naman sa baptismal ay May 13, 1980? paano at ano ang requirements na hinihinge sa local parish (kung saan bininyagan) para mag lagay ng notation sa baptismal ng correction or amended date of birth?

NSO Birh cert date of birth: April 10, 1980
I think there is a way to correct this issue, is it fot annotation? how and what are the requiement on how to process annotation?

If I understand you correctly , you will need to contact the parish priest of the church where the baptism took place .

He will have the records of the baptism .
 
So the correct date of birth is April 10, 1980 but the baptismal record says May 13, 1980. Am I understanding you properly?

If so, you should only need to contact the office of the parish where the baptism took place and tell the person in charge of records that it’s wrong. You may need to provide official proof of this, such as a government-issued birth certificate.

How the correction is noted in the sacramental record, I can’t tell you. In my parish, a recent record would simply be corrected in each of the two identical registers. If the baptism took place long enough in the past that one of the registers has already been sent to the diocesan archive, however, the process is problem different but I couldn’t tell you what it is. And each parish may have different procedures in place. Your best bet is to ask the parish of record for the baptism.

The priest of the church confirmed that the record of date of birth in the baptismal is May 13, 1980. Is there any way to correct the record as per NSO Birth Cert record April 10, 1980
 
NSO = National Statistics Office. It’s analogous to a state Bureau of Vital Statistics. When you send off for a birth cert in the US, you request it from the state, territory, or commonwealth’s BVS. An NSO is the exact same thing.

One problem is Im not able to provide them a goverment issued Id. There is a problem also with my government ID. This is the reason why I need to correct the date of birth in the baptismal certificate. I have to chaage it first and follow the NSO then next is to correct government issued Id.
 
Okay, so both your sacramental record and your official (government) record are wrong?

If so, you will need to correct the official record first and each government has their own procedures in place for this. Contact the NSO and find out what theirs are. Once your official record is correct, then proceed as I described above with the parish of record for your baptism. If it has to be done the other way around, then I don’t know. My parish will not change a birth date in a sacramental record without proof of the correct info issued by the state.

As you first posted in tagalog I assume you live here in the Philippines.
The NSO birth certificate is not the hospital birth certificate. THe NSO birth certificate is the official government one.
The details from the hospital birth certificate are used to produce the NSO one. When you get the NSO birth certificate you are supposed to check that all the details correspond to the details given in the hospital birth certificate. If you didn’t and a mistake was made in the NSO certificate it is a nightmare to get that corrected. When my step-daughter gave birth to a girl she did not check that both certificates had the same information. Years later when her daughter applied for a passport it turned out that on the NSO birth certificate it showed she was a male. This was of course a typo. Even though it was the fault of the NSO employee who made the typo they do not automatically correct it. A lawyer has to be involved and that can be expensive.

In your case if the NSO birth certificate is correct and the incorrect information is actually on your baptismal record then as others have said you need to approach the priest at the parish where your baptismal records are kept and point out the error and ask them to amend it.
 
Maybe I will ask them to annotate baptismal record with the correct date. I hopw they can amend it with notes.
 
The only way to find out is to ask them. Hope it works out for you because what @thistle describes doesn’t sound like fun…
