Can you give me some advice on returning to the church ?

Hi…
I’ll try and make this as brief as i can, as i know you’re very busy. Basically, I’m an example of lapsed Catholic. I was brought up in the church…was baptised, confirmed etc
…and went to mass weekly with my family. As is quite common, when i got to my teenage years I got lazy and stopped going. Not because of any rebellion or anything, I just stopped going…and never really returned except for Christenings and weddings. It was something i never really thought about and didn’t think about too much. I still believed in God…or at least a concept of good and evil, and I still prayed most days. To cut a long story short…i’m now 36, and have become more spiritually aware over the last couple of years. Probably starting when i got married and had such a joyous experience, then deciding to get my first child Christened…again a joyous occassion in the church. All of this made me more aware of my spirituality and renew a connection with God. I have prayed for his help on numerous occassions recently and have felt that my prayers have been answered.
I suppose I’m asking advice on what should be my next step. I want to start going to mass again and to be a practising Catholic. But what should i do ? As i said, i havn’t actively gone to mass since my early to mid teens…and i havn’t been to confession in at least as long.
I apologise for being long winded and for taking up your time. I would really appreciate any help, advice or guidence that you can offer me.
