Can the Code of Canon Law be revised or even changed entirely? If so, how and who can do it?

A new code of canon law came out in 1983, completely replacing the previous code.

Take a look at vatican.va/archive/ENG1104/__P1.HTM where you will find the Apostolic Constitution Sacrae Discipllinae Leges. I think it will answer your questions.

Smaller changes can be made along the way. Here’s an article where Edward Peters talks about a couple of changes made in 2009: canonlaw.info/2009/12/omnium-in-mentum-two-important-changes.html
