Is it possible to dedicate the merit of one’s lifelong practice to someone else? Is transference of one’s merit possible to the needs of that other person?

God is all good and all merciful. We do not know the mind of God except that which he reveals through the scriptures.

You have a dear one about to pass away? You mention Islam as your religion, if it doesn’t hurt your religious sensitivities, go to You Tube, type “Chaplet of Divine Mercy”, it’s a prayer adressed to God the Father begging mercy from him through the Passion of his dearly beloved son, our Lord Jesus Christ. It’s a great act of charity and kindness for someone to pray that Chaplet next to/for a dying person. Salam aleikoum.
