Eucharistic Adoration question

We have Eucharistic Adoration every Monday at my parish. This morning for some reason I got to wondering something. How often is the host in the monstrance(?) “changed”, or is it ever?
Don’t ask me where this question came from. It just “appeared”.

When I was an altar boy many years ago, the priest who celebrated Monday morning Mass at 8 am would consume the Host used for adoration and consecrate a new one to replace it. In this way it was kept “fresh.” This new one was in addition to the one he consecrated for his own communion during Mass. So he consumed two large hosts at Communion time,

here, once a week.
 
A church in my diocese has 24/7 adoration. I know the Blessed Sacrament in the monstrance is always the same because if you look closely you can see the image of a face.
