Extraordinary episcopal ordination

Have any canonically regular bishops received their episcopal consecration in the extraordinary form of the Roman rite since Vatican 2? If not, is there any specific rule from the Vatican that requires bishops to be ordained in the ordinary form?
 
I’m only aware of the SSPX bishops. I don’t know if there are any others (II doubt it)

Personally, I doubt there is a rule against it. However, I would guess that it is be pretty hard to find enough bishops willing & able to do it at the same place & time.

I have a follow up question- who determines in what form will it be conferred? Is the idea upon the principal celebrant or is it upon the recipient of Episcopal Ordination? Who can choose? Do they need to agree? Is there any way either side could ask for it to be done in Extraordinary Form?
 
Bishop Fernando Rifan in Brazil was consecrated in the traditional Latin Rite, with the full blessing of the Church, in 2002:

They’re both ordained in the EF, including all of the sub orders.
True, but aside from Rifan, the bishops who ordain them using the EF old rite, have been consecrated themselves in the new post-Vatican II rites (with the exception of whatever few bishops may still be living, who were consecrated in the old rite prior to 1968). Not that it makes a difference — some harder-line traditionalists would disagree — just stating the fact.

Cardinal Arinze was consecrated in 1965. My history may be a bit rusty, but I’m pretty sure that’s before the ordination revisions.

Another bishop from before the Council, Bishop William John McNaughton, just died recently. (I learned of him from this YouTube video.) He was pretty young when he was consecrated too. From his obituary, we know there’s only three other bishops alive who attended all four sessions of the Council as bishops.

I’m unaware what the exact number is regarding which bishops remain alive that were consecrated in the old rite. Whether any priests have been consecrated (with papal mandate) after the Council seems to be a matter of singularity, with Bp. Rifan’s consecration above.
 
I was trying to find that site. I figured the answer was easy to find since we have records of historically recent ordinations and consecrations. Thank you.
