From the Code of Canon Law:

Can. 920 §1. After being initiated into the Most Holy Eucharist, each of the faithful is obliged to receive holy communion at least once a year.

§2. This precept must be fulfilled during the Easter season unless it is fulfilled for a just cause at another time during the year.

No, No one is required to receive at any specific Mass, exccept the main celebrating priest who is required to receive under both forms.

No. It is only a sin to not go to Mass on a Holy Day. From the Catechism:
2180 The precept of the Church specifies the law of the Lord more precisely: "On Sundays and other holy days of obligation the faithful are bound to participate in the Mass."[117] “The precept of participating in the Mass is satisfied by assistance at a Mass which is celebrated anywhere in a Catholic rite either on the holy day or on the evening of the preceding day.”[118]
There is no requirement to receive the Eucharist at every Mass. Even if one is unable to receive the Eucharist due to mortal sin, he must still go to Mass.
