How can a divorced and remarried person receive Communion?

My mother has been divorced from my father for 38 years and remarried about 20 years ago. My parents were both Catholic and married in the church. My mother is now 68 and has never sought an annulment but would like to begin receiving communion again. Does she have to have her first marriage nullified? Can she just go to reconciliation and abstain from marital relations?
