How do Catholics view Salvation?

1.) If one were to articulate the “Plan of Salvation” to a Protestant from a Catholic viewpoint… how would it go?
2.) Is a soul who is baptised at birth into the Catholic church assured of Heaven? Does it require 1st communion?
3.) Does dying with unconfessed mortal sin assure Hell, Purgatory or neither?
