How do you get excommunicated?

I was wondering if there is like a ceremony or something that you have to undergo to be formally excommunicated.Do you get off with a warning if your about to be excommunicated?.Can it be done by any priest or only ppl of particular rank like a bishop?.Thanks for your time.
 
There are a number of offenses which warrant automatic excommunication (i.e., the excommunication is conferred by virtue of the act, without any need for formal pronouncement). These include things like procuring or participating in an abortion, enacting violence against the Pope, illicit episcopal ordinations (this is what got Arb. Lefebvre and the four bishops he consecrated into trouble), simulating priestly faculties when one isn’t a priest (i.e., in the case of invalid ordinations, as of women “priests”), and other things.

Ordinarily, however, one would be excommunicated by formal decree of a bishop. This was done, for instance, by Bp. Bruskewitz in Nebraska, who excommunicated all Catholic members of certain groups, including the SSPX, Call to Action, and the Masons.
 
To add to the above list, I believe that defiling the Holy Eucharist can also lead to ex-communication.

Ferendae sententiae (excommunication after formal proceedings):


Canon 1378 The pretended celebration of the Eucharist or of sacramental Confession.
Canon 1388 Violation of the Seal of Confession by an interpreter.

Latae sententiae (automatic excommunication):

Canon 1364 Apostasy, heresy, or schism
Canon 1367 Violation of the Sacred Species
Canon 1370 Laying violent hands on the Pope
Canon 1378 Absolution of an accomplice
Canon 1382 Episcopal consecration without authorisation from the Holy See
Canon 1388 Violation of the Seal of Confession by a Confessor
Canon 1398 Procuring an abortion
