How to distinguish apostolic tradition?

I have heard that in order to determine if something is to be deemed as apostolic tradition, we should look at the Church Fathers, however, how do we know which Church Father has the apostolic tradition? And how do we know that it is infallible?
 
Because Catholic Church is authority over that to decide.
By reading writings of Church Fathers.
Some of them:
Clement of Rome, Epistle to Corinths
St. Irenaeus, Adversus haereses
Tertulian, Apology against heresies
Cyprian of Carthage
St. Jerome, Letters
St. Augustine, Against letter of Manichaeus
