It will guide you through the easy act of perfect contritkon. Only intend to go to confession ASAP, and actually do so, and you will be back in a state of grace. There is no reason not to be in a state of grace 99% of the time, just use perfect contrition. Ofc, this implies you really mean to avoid sin, so make sure you mean to avoid it and use all ascetic means, no matter how extreme, the Church has given to do so. God bless.
You can ask God for forgiveness by your self, But, you can not be certain if God has absolved you of your sins. Only by going to confession can you be certain.
