How to present confirmation candidate for Anointing

When the sponsor comes up to the bishop he must say “Bishop ____ I present (name of candidate)_______” I’m confused as to which name it is, is it the real name, confirmation name, or the baptismal name of the candidate?

whatever it says on his nametag, the DRE or confirmation coordinator will know what to say, ordinarily you just say his given name. We have never introduced them individually in that way so I can’t say. The bishop simply reads the nametag. When I type them just add the Confirmation name if they have chosen one, so he will say,
John, be sealed with the Holy Spirit
or
John Francis Xavier, be sealed . . .
