When in a state of grace does god communicate with us and not when in sin? I need to get back to Mass and confession but when there has been a mortal sin the direct communion is cut off right? I felt more intervention when in grace and connected to some other place (reality) when in grace.

Yes, do get back to Mass and Confession! Mortal sin does not cut one off from actual graces. Otherwise, no one could seek confession after mortal sin! But mortal sin severs the state of sanctifying grace. One natural and noticeable effect ---- you seem to concur — is that things are easier when in the state of grace. Life proceeds, as it were, more gracefully!

Another thing I seem to notice is a conscious like never before. Things bother my conscious. Is this the work of the holy spirit that does not leave or sanctifying grace that can leave?

Assuming you mean conscience 😉

Your conscience bothering you about actual sins comes from God, and is actual grace. In my experience, the more I pray the rosary the stronger the voice of my conscience becomes.
