Is there a Catholic "Sinner's Prayer" available useful for evangelization?

Hi,
I was wondering if there is any good Catholic “Sinner’s Prayer”. What I am thinking is a prayer you could offer someone who is repentant and desires to reestablish a relationship with God. My idea is that ideally should be concise, mention the unlimited mercy of God, and perhaps offer a road map for the return, such as, mention the Sacraments (Confession comes to mind), or the awesome presence of the Lord in the Eucharist. And we cannot forget Mary our mother!
Do we have something like that in our Catholic arsenal?

It presents the sacraments and the necessity of the Church, but it also includes this prayer for new believers: God my Father, I believe that you have created me for no other reason than love. In a thousand ways I have shunned your love. I repent for each and every one of my sins. Please forgive me.
