If we are all going to be judged on the last day, how is it that we go to Heaven, Hell or Purgatory a soon as we die? Does that mean that those who are in Hell will be judged again? Can they then be saved?

The Catholic teaching is that there is a particular judgment at death, and a general judgment later, but without changing the outcome regarding salvation. We do not have a resurrected body at the particular judgment. The general judgment is reveals the full results of all our good and bad deeds in relation to other creatures. And " justice demands that, in order to form a proper estimate of the good or bad actions of all, a general judgment should take place" - Catholic Encylopedia
 
To add to this, the Resurrection occurs on Judgment Day, that is the physical body is raised and joined with the souls of the departed from Heaven and Hell. (The Biblical term for “resurrection” refers to a physical bodily reanimation, not merely life after death.) The body which was sown in death is raised incorruptible and, according to the Judgment, now accompanies the soul permanently at its resting place or damnation for eternity.
