Nestorius rejected that title and was condemned at the Council of Ephesus for it.
The title has been in use since the 3rd century, in the Syriac tradition (as Syriac: ܝܳܠܕܰܬ ܐܰܠܳܗܳܐ, translit. Yoldath Alloho) in the Liturgy of Mari and Addai (3rd century)[4] and the Liturgy of St James (4th century).[5] The Council of Ephesus in AD 431 decreed that Mary is the Theotokos because her son Jesus is both God and man: one divine person with two natures (divine and human) intimately and hypostatically united.[6][7]

The Sub Tuum Praesidium is the oldest preserved extant prayer/hymn to the Blessed Virgin Mary that names her as Theotokos.

Would the Holy Spirit be considered the first Person to Inspire Elizabeth (second person) to proclaim the Blessed Virgin Mary, the Mother of God?
I consider this a deep contemplative mystery over one of God’s divine revelations to our humanity, as Mary to be the Mother of God (Theotokos).
