Mass intentions

I know a Priest can offer Mass intentions and that the laity can offer up their communion for different intentions. How do you lay people do that? and do you pray along with the Priest or choose your own intentions?
 
I either mention it in my prayers before Mass or in my prayers before/after Communion.
