does it constitute grave matter to not seek medical attention when you have good reason to believe that you have a serious medical condition?
 
I have no idea, but my best guess would be yes. But then I think it would depend on the reason why someone would avoid seeking a medical diagnosis.

We have a responsibilty to take care of our bodies as well as to take care of ourselves for the sake of those who depend on and love us. Even if it is not grave matter (re:mortal sin) it is still a serious issue.

If you are in this situation yourself I urge you to seek a medical opinion of your suspected condition.

I am not sure if my answer will help or just muddy the question, but here goes:

yes and no. If there is truly nothing that can be done then no, it is not serious matter. There are some medical professionals who would put an 85-year-old woman on chemo.

Yes, if a person is purposefully avoiding treatment in the hopes that they will just die, then it would be serious matter.

I read the wording of your post of medical “attention” to mean medical intervention. If you mean avoiding seeking out any form of diagnosis due to fear, then I am not able to answer that. But hopefully someone else can.
 
From the Catechism:

2278 “Discontinuing medical procedures that are burdensome, dangerous, extraordinary, or disproportionate to the expected outcome can be legitimate; it is the refusal of ‘over-zealous’ treatment. Here one does not will to cause death; one’s inability to impede it is merely accepted.” [More not written here]

2279 “Even if death is thought imminent, the ordinary care owed to the sick person cannot be legitimately interrupted.” [More not written here]

With that in mind, we have a responsibility to care for our bodies as they are temples of the Holy Spirit. Without a diagnosis, no reasonable decisions can be made. You have to first know what you are dealing with, receive the options, then prayerfully make a decision.
