OCD and mortal sin

Hi

Is it a mortal sin if a person with OCD consents in a bad thought? Or would it be only a venial sin since the person is in a state of mental disorder?

Correct.
Mortal sin requires full knowledge and deliberate consent. A condition mitigates culpability.
 
That person needs to talk to their regular spiritual director and their mental health professional.

Those with OCD and scruples should not post here, it can worsen their situation.

Going over the nuances of these things is never fruitful. What is fruitful is listening to a trusted spiritual counselor and/or professional.
