I have a relative who had their tubes tied after her
fourth child. The reason is that she is diabetic
and each pregnancy afterward would be risking
her life. I know that under church teaching that
it is never permissible to use contraception but
then what else could she have done to avoid possible
death.
 
Use one of the many and varied methods of Natural Family Planning. When properly used, many of them are at least as effective as a tubal ligation (which, never forget, also has an inescapable risk of failure as well as do all birth control methods).

If this isn’t feasible for whatever reason, the other option is abstinence.

There are two types of death: death of the body and death of the soul.

Since the sterilization has already performed, this woman should seek the Sacrament of Confession to avoid the second type of death. Objectively, sterilization is a mortal sin. I do not presume to judge her soul one way or the other, but highly suggest she seek healing in the Confessional.

not speaking of your relative, but of the hypothetical situation. A couple confronted with that scenario has the choice of complete abstinence from intercourse until menopause, or periodic abstinence guided by one of the very effective methods of determining periodic fertility called Natural Family Planning.

Yes and every couple planning marriage must be told this age old unchangeable fact of life: there are times when sexual intercourse must be avoided for health or other good reasons involving the safety of one of the partners. There are also other situations where you will have to do without sex for a period of time–necessary business travel, military deployment, hospitalization etc. If the engaged couple can’t face that reality the should wait until they are more mature about sexuality before attempting marriage.

the hypothetical person who has relied on a temporary or permanent form of artificial contraception needs to confess and return to the sacraments for complete forgiveness.

A person with a serious/just reason to avoid having children for a time or indefinitely can use natural family planning to abstain from intercourse during fertile times, or they can choose to abstain completely.

Contraception and sterilization are instrinsically disordered-- meaning they are **always **wrong.

As the other responders have made clear, NFP is the answer.

My understanding is that your relative should confess this, even if she was unaware of the grave sin that it is at the time that she commited it. Also, I don’t believe she is required to have a reversal procedure, but I would advise she pray about it.
