Papal blessing help

My brother is getting ordained priest in May, and I wanted to get him a Papal blessing Papal Blessing Parchments | Elemosineria Apostolica
Thing is he’s in a monastic community, so he doesn’t really have a “Name” and “Surname” because he is called Brother Martin . . . (soon to be Father obviously), so how do I put that into the online form? And will Father be put on there automatically?
Secondly, in the fields that ask for “Church or Parish” and “City”, do they want his “Church or Parish” because as a monk he doesn’t have one, or the cathedral and City where the ordination will take place?

But he does have a surname. I have a friend who is a monastic, and when I address letters to her, I address her as Sr. (Religious Name) (Last Name), (Order). It’s a formal way of address. In this case, his first name will be Martin Mary and his last name is whatever his last name is.

And will Father be put on there automatically?

I’m not sure, but since it’s for an ordination, I’m sure they’ll address it correctly.

It would be the cathedral where this will take place.
