Are you certain of that?

The reason why I ask is that it’s customary to put the book on the altar during the month of November and especially on All Souls Day. Have you ever noticed such a book?
 
Yes. I believe my parish does this. There is also one set up in our 24/7/365 Perpetual Adoration Chapel.
 
I have seen these books. Not all parishes have them though, and in some cases you can write other intentions in there, not just for deceased.
Perpetual Adoration often seems to have an intention book.
I do put the most recent deceased persons in need of help that I know in every book I can find.
I also put in “for the Forgotten Dead”.

I’m sorry to hear you don’t have one. Not all parishes have them. I do know that many places stopped using them, but I’ve also noticed a certain restoration in recent years.

I looked at your location in your profile (not hard to guess for someone with a username that starts with Cajun after all!) and I thought that would be a custom that was maintained strongly there.

Maybe you can suggest to your pastor to bring it back or start one?
 
It can be “any” book. I’ve seen many variations.
At the top of the list, I’ve seen pre-printed books with nice covers made specifically for that purpose. I’ve seen these in religious goods stores/catalogues but they’re not easy to find.

At the other extreme, I’ve seen just simple “blank line” or “record” books from stationary stores or even student notebooks. The last one just looks cheap.

We use (1 at each location) hardbound books that are plain blank lines on the inside while the cover has the church names stamped on them along with the words “Faithful Departed” The inside has nothing but names: no dates or other comments.

The book only needs to have blank line–no columns, just lines. I’ve even seen completely blank pages, but don’t recommend it because people will write in different sizes and it looks disorganized or maybe “messy” might be a word. Lots of wasted space.

Over the years (being transferred a few times) I’ve had several occasions to order these for the parish. They’re not expensive. Here’s one example www (dot) bookfactory (dot) com. I’m only posting that to illustrate how easy it is to order such a book. No endorsement implied.
