I was received into the Catholic church years ago. Later I was received into the Episcopal Church. Years later I registered in a Catholic Church and became active.

Am I Catholic from the standpoint of canon law? Should I have made an act of faith or something?
 
You need to go to confession to confess that you left the Church. Then everything will be good.

You don’t need to make an Act of Faith, I believe, but you do need to complete (if you didn’t before your move to Episcopalianism) the Catholic sacraments of initiation (baptism, First Holy Communion, Confirmation).

In addition you need to go to confesion, and confess the sin of having spent time in another faith.

Beyond that there’s not really anything further you need to do, although you may wish to go to RCIA to learn more about the faith (its not just for those preparing to enter the Church).

But you might want to check with your priest just in case, since I don’t know all the details of your situation and there may be something I’ve missed here.

You need to go speak with the pastor of the local Catholic church. By being “Received” by formal act into the Episcopal Community you left the Catholic communion.
