We can’t see the bead. However, it is not uncommon for Rosaries to have an extra medal around the medallion somewhere. Some people have multiple extra medals there.
It helps some with “usual rosary intentions” before the first mystery/decade. Maybe a little dedication to a saint tossed in there too.

I have medals on most of mine which have been attached as extras to the rosary. On has the St Benedict medal, others have patron saints, others again have the Sacred heart/immaculate heart of Mary. They just help me to remember to offer up additional prayers.
