Switching Rites and Code of Canon Law

Code of Canon Law (for the West) states that for one to be enrolled in another sui iuris church one needs the Holy See’s permission (Can. 112 §1, no. 1.) However, from what I was told and read one only needs the bishop’s permission of the eparchy you wish to join to accept you into it and to have your current bishop’s permission to release you to the eparchy you are trying to join. Can someone clarify which one it is?
 
Older rules required that it go to Rome. Today, it can be done between the two bishops (shipping & receiving).

There was a requirement that it still go to Rome if there was an ordination in the future; I just don’t know if that is still there.

The permission of the Holy See is presumed if both bishops agree.
 
I know that the Code of Canon Law for the Eastern Catholic Churches states this. Is the Holy See’s permission presumed if both of the bishops agree because of the Eastern Churches’ law? If not is there a document that the Holy See issued clarifying that the same applies to Roman Catholics?

As the eastern code is an (improper) imposition from Rome, it states Rome’s consent.

Yes, there was a rescript of the Latin CIC shortly after the CCEO was promulgated. Recently we have De concordia inter Codices (2016.05.31) which clarified several Latin CIC canons, so they are like the eastern CCEO canons.
