My question is pretty simple. What should you do if you think you might be in a state of serious sin, but are really not sure (say that you don’t know if all 3 requirements for a mortal sin were met)?

Just go to confession and confess you sin, that is the best way to release it, make it a good one.

The Church encourages confession of all sins, both mortal and venial. You don’t have to be sure which it was in order to be forgiven.

I would go to confession and confess the sin.
 
I agree with the other posters. When I find myself in this predicament, I abstain from the Eucharist at Mass and go to confession as soon as possible. I’m also of the opinion that a regular confession is very healthy. I try to go once a month no matter what.
