Toddler grandson - legal question

I would appreciate some suggestions for our situation.
My 3 and a half year old grandson lived with his mom and other grandma, a mile from us. My son who is the child’s father lives with my husband and I.

We just learned Mom has gone to live with new boyfriend and visits her son maybe twice a week for an evening. Other grandma doesn’t speak much English, isn’t helpful with potty training, I suspect doesn’t interact with grandson much as she caretakes 3 other toddlers.

Grandson is delayed in speech, maybe because he doesn’t get much English practice.
Son wants him to come live with us, and since he works 2 jobs, I would be doing much of the care taking, which is okay with me – the child needs consistency.

My concern is, if I’m going to sign him up for nursery school (for remedial speech – (already prescribed by MD a year ago but not started) & take him to doctor, etc, what kind of permission/paperwork would I need?
Mom has said she doesn’t want to share custody, but she’s okay with her son living with us. I’m told she does cocaine, but only on weekends; she is sometimes belligerent, occasionally violent toward the baby’s father.

Yes!

I strongly suspect that there is some normal paperwork for being able to take a child to the doctor when not the guardian. Otherwise, how could a nanny take a child to the doctor?
