What is needed for salvation?

I have heard that you only need to die in a state of grace to go to heaven (or purgatory), but have become confused over the Bible verse where Jesus spits out the lukewarm. I feel lukewarm but am also confident I am not in mortal sin…so what is needed for salvation?
