What does “real presence” refer to?
I know what transubstantiation refers to but this term is confusing. An internet search didnt help with with a definition.

Christ is really present in the Eucharist. When the Eucharist is present, Christ is present. The bread and wine change to the body, blood, soul, and divinity of Christ, such that he is present as a person.
 
“Sacramentally present” is the term used in the CCC:

1353. In the epiclesis , the Church asks the Father to send his Holy Spirit (or the power of his blessing180) on the bread and wine, so that by his power they may become the body and blood of Jesus Christ and so that those who take part in the Eucharist may be one body and one spirit (some liturgical traditions put the epiclesis after the anamnesis).

In the institution narrative , the power of the words and the action of Christ, and the power of the Holy Spirit, make sacramentally present under the species of bread and wine Christ’s body and blood, his sacrifice offered on the cross once for all.

An internet search didnt help with with a definition.
It means to you whatever you want it to mean. If you think it is bread and wine, then that is what it is. If you believe it to be a symbol, then it is a symbol. If you believe it to be the true body and blood of our Lord, then I believe you receive the true gift.
This isn’t Catholic teaching. The Real Presence is there regardless of whether we believe it is there or not.

Yes and no. The innermost reality of the bread and wine, the ‘substance’ changes, the accidents (how the bread looks, tastes etc) remain.

The Real Presence is when Jesus’'s Body and Blood are present sacramentally, in a new and unique form of Presence.
 
The Real Presence is The Real Jesus present before you in the wine and/or bread (wafer) given to you when you participate in the sacrament of the Eucharist.
 
At the end of the day, it is a mystery. So rather than try to understand it, I focus on deriving the spiritual benefit from it and being grateful for God’s wonderful gift.
 
Some people think it easier to understand in this way: “Our Lord Jesus Christ, present under the appearance of common bread and wine”.
