When was the doctrine "transubstantiation" established?

I would like to know when the doctrine of transubstantiation was actually established. In other words, when did it become a fact that the communion host is transformed at the Mass into the living body of Jesus Christ? I thought it was at the Fourth Lateran Council in 1252 but a Catholic friend of mine is telling me that it has been that way from the beginning.
