Jesus is referring to categories laid out in Jewish law. To be “born a eunuch” as opposed to being made a eunuch (i.e. castration) meant that the man was either impotent or infertile. Either situation would be problematic for marriage in the ancient world but being naturally that way as opposed to making oneself that way were judged differently. It doesn’t necessarily mean the man was like that from birth.
