Who is Our Lady?

I have often wondered, “Who is Our Lady”? I know there are lots of ladies, but who are they and why are the addressed as “Our Lady of [insert name]”?
 
Our Lady is the Blessed Virgin Mary, Mother of Jesus Christ, God the Son. Her various titles (e.g., Our Lady of Fatima, Our Lady of Perpetual Help, etc.) generally indicate either places where she has made an appearance in a private revelation or specific causes for which she is asked to intercede.
