Appointing Bishops

Not sure if this is the right forum but what is the process for picking Bishops? I know it starts with the Nuncio but why are we left with empty sees?

Here in NY the Diocese of Rochester has no Bishop and in in Albany (my diocese) Bp. Hubbard turns 75 this year. They (whoever they are) should have had coadjutors in place ready to go.

What gives?
