I think it would be best to use your official (PSA) name on everything, to the extent possible. I think you may only need the baptismal name when the church needs to look up records of your sacraments. For example, to be married in the Church, they may wish to check that you were baptized in the Church, and so they would need your baptismal name.

I made the mistake of using my Confirmation name when I applied for my Social Security Number (United States), and this caused a lot of trouble for me later on. I should have used the name on my official birth certificate. Since then, the US government has changed the rules so that it is impossible to use any name but the one on your birth certificate.
