PSA - Philippine Statistic AuthorityI believe that PSA and NSO in this context refer to the birth record (or birth certificate) in the Philippines.

So I surmise that the OP’s name on the baptismal certificate does not match that on the birth record or certificate.
I think it would be best to use your official (PSA) name on everything, to the extent possible. I think you may only need the baptismal name when the church needs to look up records of your sacraments. For example, to be married in the Church, they may wish to check that you were baptized in the Church, and so they would need your baptismal name.but im using now my new psa
