Can you explain the commission for EMHC?

Is there a standard time for Commissions when you are a extraordinary Minister of Communion? Does it vary Diocese to Diocese? What happens to the dates when you are commissioned in two venues, parish and hospital through the Chaplain?

Local Ordinaries possess the faculty enabling them to permit fit persons, each chosen by name as a special minister, in a given instance or for a set period or even permanently, to give communion to themselves and others of the faithful and to carry it to the sick residing at home…
It varies from diocese to diocese how long a period of time EMHC are commissioned. Once the time period of the commission expires then the individual no longer functions as an EMHC (unless appointed for a one time situation).
Usually an EMHC will be commissioned for a specific time frame in a specific place (e.g. a parish or a hospital). So if John is commissioned for 2 years at his parish and 3 years at a hospital that means after 2 years he is no longer to serve as an EMHC at his parish liturgies but may continue to do so at the hospital for another year.
