Choosing bishops in previous eras

It is my understanding that the process of having the Holy Father choose bishops is relatively recent, only having been done over the last couple centuries (correct me if I’m wrong). Before that, how were Bishops chosen (aside from public acclamation)? And what measures were taken to ensure that unworthy bishops weren’t chosen?

Long long time ago the Bishops were identified by their charismatic gifts. As successors to the Apostles they were expected to show those gifts. However, you must remember that the Church always had bishops that created problems (e.g. heresies).

In many cases they were chosen by the local lord or royalty. Check out Lay Investiture. As I recall the compromise was that the local lord or king could name the candidate, but he had to be installed by a Church Authority. [Look at the current problem in China.]

As noted there was a period of popular acclamation. Check out how St Ambrose was still a catechumen when chosen as bishop.

At other times I think they were elected by the local clergy. Note that the Cardinals, as elect the Bishop of Rome [aka the Pope] because they hold office as the bishops, priests and deacons of Rome.
