Confession and the mentally ill

I’d like to know under what cases is someone with a mental illness unable to confess? If someone was out of touch with reality much of the time, not sure whether they really did certain things or not, and have previously accused someone of something they realized when their thoughts were more sane and fluid later, did not in fact occur, how could one deal with this and how would the Catholic church community and pastor need to respond?
