Confirmation Certificate after name change

I had my name legally changed. My Baptismal Certificate reflects my old name. When I go for Confirmation, will the Confirmation Certificate be with my old name or my new name?

My Baptismal Parish says they won’t change the name on their records. So since the Confirmation Parish would need to send their certificate of my being Confirmed back to the Baptismal Parish so my records could be annotated, would it be correct to assume that the Confirming Parish would do it all under my old legal name which is on my Baptismal Certificate?
 
We had a similar situation in our parish where a student had been adopted and his name was changed as part of the adoption.

The Confirmation Certificate must be issued in the Baptismal name.
The Baptismal record is annotated (but not changed) to reflect the name change.
The official record of all Sacraments is the Baptismal record so that would include the whole “paper trail” of original name and name change.

At least that’s how we were instructed in our diocese.

If a previously baptized child is adopted the baptismal records are annotated and a certificate is issued with the new name and the names of the adoptive parents. All the original information remains in the records.

I’m confused about the baptismal parish’s stance. I’ve seen baptismal records annotated to reflect a legal name change and a new certificate issued with the new name.
