Humble opinion as a former DRE of a large parish (we Confirmed 125 my first year and 170 my second year): you are doing too much. I like all of the ideas behind your requirements, but they seem too much. I always preferred less Confirmation classes, no hours attached to service projects (I required 2 services activities with reflection papers that we discussed instead of hours), and encouraging all to receive the sacrament. Ideally, we asked for the 2 years of Catechesis before Confirmation, but by the books we should not deny Confirmation to someone properly disposed who asks for it (in my case, these were kids and parents asking about Confirmation 2 weeks into their 8th grade school year when Confirmation was spring of that same school year).
If your program is working for you, God bless you! But if you are finding widespread legalism and disillusioned parents because of the “hoops” to be jumped through, I would really consider a simplification of the process. Less hour counting for you, too!
