I’ve not heard the difference between “formed” and “informed” emphasized. I suppose an informed conscience is one that has been formed well through solid religious education and/or reading and has been helped with the grace of regular partaking of the sacraments.
I’m not sure if you’re asking as a way of getting some ideas about how to better form one’s conscience? A priest should be able to recommend some good spiritual reading and/or videos. Or maybe you could start with reading some of the doctors of the church.
