If a baptized Catholic decides to marry another who is not baptized and they plan on a wedding in the Church, does the couple have to sign paperwork for a dispensation from the bishop or is that just the priest’s responsibility?

in general the priest preparing the couple for marriage arranges, as part of the paperwork, for any needed dispensation. you should ask about it in case the situation has been overlooked, just as you would double check on other aspects of your preparation for marriage. the priest should have already discussed this with you, since the non-CAtholic party must be informed of the duties of the Catholic party to the marriage.

I’m looking at the form my diocese uses:

FORM FOR PETITION FOR DISPENSATION FROM DISPARITY OF WORSHIP or DISPARITY OF WORSHIP (ad cautelam)

The priest fills it in giving all the information on both the bride and groom.

The Catholic party signs the 'Declaration of Catholic" which says:

Recognizing the gift of my Catholic Faith, I intend to live it and preserve it. I promise to do all in my power to have all the children baptized and brought up in the Catholic Church.​

The priest then fills out and signs the “Declaration of the Priest” section and forwards the whole thing to the Bishop for his consent or rejection

A dispensation for disparity of worship (cult) is necessary to marry a non-baptized person.
A dispensation for lack of canonical form is necessary to marry in a non-Catholic Church or in a civil wedding.
Permission is necessary for a mixed-marriage (marrying a non-Catholic baptized person). In my diocese that permission can be granted by the priest.

Permission to wed outside the Church is a separate dispensation.
