Do patients have to fast for one hour before Communion?

I’m one of the volunteer pastoral care ministers who brings Communion to the local hospitals and nursing homes. My work schedule doesn’t allow me to go to the hospitals and nursing homes at a specific time every time. Sometimes, I come right after lunch or after snack time. I’m wondering if the patients are subject to the same one hour fasting rule before receiving Communion?
 
A person who is ill or for whom a fast would otherwise be a serious hardship is not expected to observe the one-hour fast because, for that person, the food can be considered medicine.
