Falsified Documentation to Replace Genuine Articles... Perjury?

So, in my state (Illinois), it is mandatory for drivers under 18 to have 50 hours of driving under their belt before applying for a license (ten of which have to be at night… I’ll get back to that). The vast majority of the people I have talked to falsified their driving records, pretending to do far more hours then they actually have. I had completed about 40 hours when I lost my driving log. After that event, I tried keeping logs on a piece of paper… and I’m pretty sure that I completed all 50 (although I’m not sure if I completed 10 of those hours at night).

Anyways, since I didn’t have the previous 40 hours recorded (I had obviously forgotten the exact details of each entry), so I made fictitious entries in a new copy of the log so that I could apply for my license. At the DMV, I signed a document stating that “under penalty of perjury” none of my documentation was false. Obviously, my log was!

I drove all of the hours (thereby obeying our state law)… but my documentation itself was falsified. Did I commit a sin in my actions? And if so… how serious was it?

As a side-question, although I drove the manditory 50 hours, since I lost my log, I’m not quite sure if 10 of them were at night. Of my final 10, I am sure that 6 of them were at night, so its a pretty good guess that I had it… but once again, I’m not sure…
 
I would say that at the time you should have been up front to the DMV about what happened… in the degree of the sin I could only say a confessor could tell you though I imagine for something like this expecially since in most instances you were trying to be honest, I highly doubt this would be considered a mortal sin. I would say confess it, but I doubt you need to do much more since you probably did do the required driving. Listen to what a confessor advises.
