Friends,
I would like to give my son a medal for his first communion, however I don’t know if that is something that is appropriate for a seven year old boy.
Other suggestions are appreciated.

Does your parish do a Scapular investment as part of the First Communion celebration? Ours did, so I got my son a scapular medal. Many of his friends have medals of patron saints.

Or Google “first communion banner” for patterns that you can make with a trip with your boy to the local.craft store.

If you are crafty and do home projects, this may be fun for the both of you to hang in his room.

How about a crucifix?
 
I got the first 2 of my nephews a leather/bound Bible for 1HC with their name embossed on the cover.

Will probably do the same for the next this year.

The idea of a medal is cool…a Miraculous medal, particularly…but check to see if his school permits wearing a chain of any sort. Our schools here discourage it due to the rigors of Phys Ed class and the playground. I guess they are afraid of getting sued. 🤷
I would be disappointed to receive a lovely medal and not be able to wear it daily.
My favorite gift was a statue of the Blessed Virgin holding the Child Jesus. I found it comforting on my nightstand when I had bad dreams. She looked so beautiful. I still treasure it.

Our family tradition is to give a Mass Book, Bible, Rosary, and Crucifix. The Mass Book is great for following along during Mass, as well as learning about it at home. It contains the Stations of the Cross, Rosary, and many other prayers. It also helps to explain what we do at Mass and why. Some other ideas are a Patron Saint statue, holy water font, Miraculous Medal, or stuffed saint doll. If he is particularly fond of a saint, or if you would like to encourage him to learn about the saints, the stuffed saints would be a good idea. They are available on Etsy. Also, saint books are available at most Catholic book stores.

Also appropriate are:
Rosary
Crucifix
Bible
Missal
Book on the saints
Prayer Book

Many Catholic gift stores have sets that are designed as gifts for 1st Communion. The sets include a missal, rosary, scapular and a pin.

Also check to see if the parish is gifting anything. Our parish gives a prayer book. You wouldn’t want to give your son the same thing the parish gives.

I might also give him one of the prayer pillowcases.
