Fivefold Scapular

Does anybody know where I can find the Rite of Enrollment in the Fivefold Scapular on the Internet? I asked my pastor, and he said that his Book of Blessings only contained the Rite of Enrollment for the Scapular of Our Lady of Mount Carmel. He said that if I could find the Rite for the Fivefold, he would enroll me in it. This page must have the authorized and official version of the Rite, of course.
